Eligibility for Services

Miami-Dade County receives funding through various federal and state programs for the provision of medical and social support services to individuals with HIV/AIDS. For most services, individuals must meet the following eligibility requirements;
  1. Be HIV positive or have AIDS
  2. Have an income that does not exceed 300% of the Federal Poverty Level
  3. Be a resident of Miami-Dade County
If you meet the criteria listed above, you are eligible for most HIV/AIDS services provided by these programs.

How to Apply for Services

The best way to access services is to contact an agency that provides case management. Your case manager can then direct your care across the service network. In order to assist you and to ensure access to those services to which you are entitled, the case manager will collect documentation from you to prove that you are eligible for services. In most cases, the case manager will then be able to refer you to other services using a certified referral form. This eliminates the need for you to repeatedly produce paper documents to prove that you are eligible for services. Listed below are some of the documents you will be asked to bring to the case manager – be aware that some agencies may ask for more documents but most agencies will ask for the documents listed below. Prior to your appointment at the case management agency you select, it is a good idea to call and ask what documents you will be required to bring.
  1. Proof of HIV
  2. Proof of income from employment
  3. Proof of Miami-Dade County residency
  4. Green card, visa, passport, immigration, naturalization papers (Not a requirement for Ryan White services*)
  5. Social Security numbers for all household members*
  6. Rent receipts, canceled checks from landlord, eviction papers
  7. Utility bills, monthly expenses
  8. Photo Identification
  9. Voter registration
  10. Bank statements
  11. Life, health and burial insurance policies
  12. Auto title and registration
  13. Deed, mortgage or tax papers for own home
  14. Military discharge papers
  15. Birth certificate
  16. Proof of marriage, divorce
  17. Medical records and labs (for transfer clients)
* You do NOT have to be a legal resident in order to receive Ryan White services; you DO, however, need to be a resident of Miami-Dade County.
