How to Apply for Services
The best way to access services is to contact an agency that provides case management. Your case manager
can then direct your care across the service network. In order to assist you and to ensure access to those
services to which you are entitled, the case manager will collect documentation from you to prove that you
are eligible for services. In most cases, the case manager will then be able to refer you to other services
using a certified referral form. This eliminates the need for you to repeatedly produce paper documents to
prove that you are eligible for services. Listed below are some of the documents you will be asked to bring
to the case manager – be aware that some agencies may ask for more documents but most agencies will ask for
the documents listed below. Prior to your appointment at the case management agency you select, it is a good
idea to call and ask what documents you will be required to bring.
- Proof of HIV
- Proof of income from employment
- Proof of Miami-Dade County residency
- Green card, visa, passport, immigration, naturalization papers (Not a requirement for Ryan White
services*)
- Social Security numbers for all household members*
- Rent receipts, canceled checks from landlord, eviction papers
- Utility bills, monthly expenses
- Photo Identification
- Voter registration
- Bank statements
- Life, health and burial insurance policies
- Auto title and registration
- Deed, mortgage or tax papers for own home
- Military discharge papers
- Birth certificate
- Proof of marriage, divorce
- Medical records and labs (for transfer clients)
* You do NOT have to be a legal resident in order to receive Ryan White services; you DO, however, need to
be a resident of Miami-Dade County.
